Blue Agilis 2023 Year in Review Highlights

In the transition from FFS to VBC, AWVs are the cornerstone of achieving optimal patient outcomes. However, providers are often unsure how to best change their workflows to enhance their performance and maximize their preventative care reimbursements. In 2023, Blue Agilis tools helped provider groups achieve diabetes, breast cancer, and cognitive decline goals with a 6 month improvement from previous years.

17% Reduction in 30-day unnecessary readmissions:

Working with Blue Agilis, one large health system was able to optimize their discharge to home process, improving post-hospitalization recovery for patients with congestive heart failure. This initiative led to a proactive approach for addressing potential complications that could have resulted in an avoidable readmission.
